I ________________________ the undersigned participating individual or parent/guardian of this student, do hereby authorize the directors, employees, assigns, and teachers of en Taiko as agents for the undersigned or myself to consent to secure proper treatment for myself/my child in a medical emergency. I hereby release and discharge en Taiko from any and all claims for personal injuries. I/my child are physically fit to practice and engage in the intensive physical program normally connected with Taiko training and there is no reason known to me/my child for not participating in these activities. I agree that photographs, audio or video recordings of me/my child may be used for promotional purposes by en Taiko. This agreement shall remain in effect until I have actively ceased any activity with en Taiko and have also given 30 days written notification of intent of being released from this wavier. I agree that en Taiko has permission to contact me in the future at the above mailing address, phone number and email address. I also authorize en Taiko local emergency services in order to secure proper treatment for my child as named above. I release en Taiko from all liability or responsibility for any accident or injury to my child. For the safety of all students, the drums and other equipment, each student is expected to follow the rules and the teacher’s directions. During each class the consequences will be: One verbal reminder or warning The second reminder or warning will include a 10 minute time out The third reminder or warning will include removal from this lesson and a call home If a student is removed from two classes, that student will not be allowed to finish the rest of the classes, with no refund of any tuition --REFUND POLICY: Class registration withdrawal requests that are received two weeks prior to the start date of the class are eligible for a full refund minus a $50 processing fee. Requests received after that time will be credited to your next class registration. Workshop refund requests are only eligible for credit. Credit expires one year from date of issue. All refunds and credits are subject to en Taiko’s approval. Late fee will be charged after due date. en Taiko will not be refund or credit for the cancellation after the class starts. One verbal reminder or warning If you have not follow these rules, you may lose your spots from the songs or not able to join the performances. After 3 warning, you may removed from en Taiko If a student is removed from two classes, that student will not be allowed to finish the rest of the classes, with no refund of any tuition
